True…I also jumped right in with + 150 mods combined, have enough experience with Bethesda that I no longer bother playing "vanilla", even on the first run.
I haven't played a Bethesda game vanilla since Morrowind.

Weeeeell…maybe exaggerating on the number there, but enough that I would probably barely even recognize "vanilla" game. One thing that stood out, were quest mods…surprisingly good, better than any in game at least.
What I'd really like to see in F4, is a massive, well designed city…sort of your in game hud, where characters change, quests progress throughout the game…still remember the atmosphere and design of Gothic II Khorinis, but I'll be damned if I can remember the name of even one from it's sequel and all Risen games combined. Biggest letdown of New Vegas, in my opinion, as well.
They should really look up to Wild Hunt Novigrad as an example.
