I had heard good things about it and tried it a few years ago. My main issue was that the beginning if the game is really slow and repetitive, which is a problem when you die so much. Good Pacing is really important in roguelikes and for me this just didn’t have it. But I’m not sure if it might be better now.

I don't know how long ago they were added as I'm new to the game, but when you start you can choose the usual Joppa start, or any of four others. So there is a way to vary the start, though my impression is that the other four are all more difficult.
Much is the same, but there are also randomized elements, as in any roguelike. I've played the start around half a dozen times and there have been differences each time. Different enemies, different ruins, different stuff to find. In a map where once there may have been nothing but watervines and crocs, another time there might be ruins with apple farmers' daughters with which to be smitten (one of my favorite things I've encountered so far)

The writing is evocative and lovely. Here's a cat, Ctesiphus, from the opening village of Joppa:
One star-ribboned night, a ray cat crossed the marshy loam and wandered into Joppa. The watervine farmers, wakened by the joyful cries of a small girl, gathered around to revel at the favorable omen and extol the generosity of the Beetle Moon. Since then, Ctesiphus has spent his days curled under the shade of brinestalk huts and sauntering over dirt paths. So long as he's approached with care, he welcomes the hands of friends and strangers alike.
