I see. I guess that's what Hell is, a difficulty level? But it's a place in Diablo II, too! I mean, read stuff like: I can solo Hell with this character on level 42! It could be both! I'm confused! 
Anyway, I think I'm gonna need Lord of Destruction for the scalable automap, because I can't find a way to do that.

On Hell - difficulty.
In Hell - the place.
When they're talking about just "doing Hell" it's usually the difficulty.
