Merchants out of cash?

Do they actually get more money or are they out of cash forever, seeing as they don't seem to get new equipment, at least during chapter 1, well maybe some +1 arrows respawn.

Or is this just another bug or poor design?

You'll get to vendors later, especially the ones you can get at your stronghold, with huge amounts of money. They can still run out, but it takes a lot. It's not really an issue after the end of Act 2.

It also appears that 'spending limits' are still there - so that if you have a very rare item you won't get very high value early on.

This happens in G3 and it annoys me there too!!

*Opens purse and starts saving every coin for now.*
