I also got Reclusive Tactician and Immersion Engineer.
Looks like many of the questions are looking specifically at addiction rather than just gaming in general.
Was a bit of fun filling it out.
One thing that amused - the survey listed Zombies, Run! as a GPS game along with Pokemon Go.
I use Zombies, Run! regularly and it isn't a game unless you're being extremely broad with your definition of game - it'd be much better described as an audio story. I'm a character in the story, but the story tells me what my character is doing and the listener never gets to decide what to do.
It still is a fun way to get exercise as I get extremely bored walking/running without the reward of entertainment! Not dying of heart disease isn't enough motivation for me - tell me a story, damnit!
